Social Prescriber
We reserve the right to withdraw this vacancy early should we receive a large number of applications; therefore, we suggest you do not wait until the closing date to submit an application. Interviews may take place before the closing date to facilitate a quick hiring process.
We have been awarded the contract for the Wrexham-wide Social Prescribing and Community Wellness Service, commissioned by Wrexham County Borough Council (WCBC) in partnership with Betsi Cadwaladr University Health Board (BCUHB), for residents aged 25 and over. It builds on what already works and is delivered with Citizens Advice Wrexham as our specialist advice partner, addressing the needs of residents across the whole of the County.
Social Prescribing connects people with non-clinical support for the social, economic and environmental factors affecting their health. Our System Ready model works around the person: psychologically and trauma-informed practice; practitioners embedded in GP practices and neighbourhoods; and integrated working across health, social care and the voluntary sector. The service is non-clinical and preventative – it does not provide counselling, crisis response or clinical treatment.
The new service brings more outreach into rural communities and areas of highest need, Citizens Advice integrated into the pathway, and a presence at the Wrexham Wellbeing Hub, Adult SPOA and Wrexham Maelor Hospital. We follow the nationally aligned pathway – What Matters conversation, co-produced wellbeing plan, warm handovers, review and planned closure – in line with the National Framework for Social Prescribing and the HEIW Competency Framework.
Role Responsibilities:
Referrals and access
- Receive self-referrals and professional referrals (GP practices; BCUHB teams including Wrexham Maelor, the Emergency Department and Minor Injuries Units; Adult SPOA; Adult Social Care; housing and community partners) – a ‘no wrong door’ approach.
- Maintain a presence in aligned GP practices and cluster triage/MDT meetings, and support the service at the Wrexham Wellbeing Hub and Adult SPOA, including step-down from statutory services.
- Offer pre-referral advice to professionals and keep referrers and GPs (with consent) updated, including a closure summary.
- Deliver support in trusted community venues and through outreach into rural communities and areas of highest deprivation with flexible, non-digital and home-visit options where appropriate and safe.
Person-centred support
- Hold structured ‘What Matters’ conversations and co-produce strengths-based wellbeing plans.
- Work in a trauma-informed way within the boundaries of a non-clinical service, escalating promptly where specialist clinical, safeguarding or urgent needs are identified.
- Provide multi-contact support, review and planned closure, with warm handovers and supported introductions to community groups, peer support, volunteering, learning and activities.
- Address the wider determinants of wellbeing (housing, income, employment, transport, caring), referring to Citizens Advice for debt, benefits, housing or legal issues while retaining case ownership, and to community outreach for practical support.
- Promote statutory advocacy, and make reasonable adjustments for people with additional needs, including dementia, caring roles, neurodivergence, learning disability or sensory impairment.
- Deliver the Welsh Language Active Offer and arrange translation for other community languages where needed.
Partnership and community connection
- Build effective relationships with GP practices and clusters, WCBC, BCUHB, AVOW, Citizens Advice and networks such as Ageing Well Wrexham and the Wrexham Poverty Action Collaborative.
- Use DEWIS Cymru as the primary directory of community assets and help keep it up to date.
- Identify gaps and share evidence so that partners can develop community-led responses (the service enables, rather than directly delivers, community activities).
- Promote the service across Wrexham, with a focus on seldom-heard and under-represented groups.
Recording, outcomes and quality
- Keep accurate, timely records on Elemental (and Mosaic where required), including equality data and Welsh language preference.
- Complete outcome measures (SWEMWBS, ONS4, UCLA Loneliness Scale) and contribute statistics and case studies to evaluation and contract reporting.
- Handle information securely under UK GDPR, the Data Protection Act 2018 and WASPI, using approved secure channels, and AI tools only as permitted by Rainbow policy.
- Manage your own caseload safely, raising capacity or risk concerns with your manager early.
- Follow lone-working and health and safety protocols for venues, outreach and home visits.
Other duties:
- Carries out additional tasks that may be required from time to time to achieve agreed service outcomes
- Completes mandatory and commissioner-required training before independent practice, with annual refreshers.
- Takes part in regular supervision, reflective practice, case-file audit and annual appraisal, with an Individual Learning Plan.
- Maintains confidentiality of all information and records relating to the work of the Rainbow Foundation and work undertaken for monitoring and evaluation purposes.
