With one in four individuals experiencing mental health challenges, the Rainbow Foundation is delighted to announce that its Enhanced Social Prescribing service, based in GP practices across Wrexham, has been awarded the title of Best Mental Health Support Service at the Mental Health and Wellbeing Wales Awards. This recognition highlights the team’s impactful work in providing talking therapy and tailored mental health support to individuals and communities.

“Changing Habits for Life” is a FREE social, psychological and physical behaviour change Programme. Delivered through a combination of extensive tests, customised coaching, lifestyle advice, and peer support, the programme will support participants as they change their habits during the 66-day programme.

Just seven months ago, Jeffrey Chrimes could not walk the distance of his yard without a risk of falling.
